How it Works

Mobile Health Education Modules (MHEd Modules) are short, accessible patient education sessions designed for specific health conditions. The MHEd Modules are delivered by Community Paramedics and are designed to help patients improve their health literacy, better understand their health condition, lifestyle factors, complications and how to manage symptoms. Community Paramedics will bring their mobile device to home visits and share resources such as videos, images and animations related to the patient's health condition. The patient will also receive handouts and online resources to review during and after their session. Documentation can be integrated directly into your documentation software, allowing for seamless coordination across your CP team.
